The Booking Request Form is a way to communicate initial information about your presentation to SMARTRISK’s Program Coordinator. Expect a response to your request within 3 business days following submission.

Prior to filling out the form, ensure that you have an idea of venue and audience size. If you are unsure that your venue meets necessary requirements for the show, review the audio-visual and accessibility page here.
